Check out the panels below or search for your lab tests on our website. Add them to your cart, register for an account, checkout, and get your order confirmation emailed to you.
Visit a Local Testing Center
Search by zip code to find the closest Test Centers to you. Print and take your requisition to one of our approved lab locations at your convenience to have your tests performed.
Share Results with
Equal Health
Most test results are available within 5-7 business days and stored securely in your account on Grassroots Labs. Send your results to Equal Health via the "Share" feature in your account profile on Grassroots Labs using the following email:

Just download your lab order after checking out and visit your nearest approved testing center.
Want to Schedule an Appointment at the Testing Center?
You are not required to have an appointment for most testing when visiting the test center, but if you would like one, click below to visit our lab vendor, Quest Diagnostic's website to schedule an appointment.
Share Your Results with Your Equal Health Provider
Your results should be ready in approximately 5-7 business days. Once your results are in, you will be notified by email.
You have two options to share your results with Equal Health: upload your results to your Equal Health provider in your Equal Health member portal or share directly from your Grassroots Labs account via encrypted email.

Sharing Results Instructions
Option 1
Download Your Results and Upload them to Your Equal Health Member Portal
Log in to your Equal Health Member Portal below:
Click on "My Portal".
Once logged in, scroll down on the Home Page to "Messages" and send a Message with the Lab results as an Attachment.
Your Equal Health Medical Provider will reach out via your Equal Health Member Portal within 3 business days.
We appreciate your patience as your Equal Health medical providers take time to review your results!

Share from Your Grassroots Labs Account via Encrypted Email
